Apply for a Passport

Who May Apply for a Passport

A U.S. passport may be issued to a citizen or national of the United States. Unless specifically authorized by a passport issuing office, no person may have more than one valid or potentially valid U.S. passport of the same type at any one time.

Required Application Form

Passport Application Form DS-11 must be completed and executed in person before a passport agent by an applicant applying for a passport for the first time or a person not otherwise eligible to use Form DS-82, “Application for Passport by Mail”.  

The following items must be submitted when applying for a U.S. Passport:

  • Completed passport application (DS-11) in black ink.
  • Certified copy of birth certificate.  To obtain a certified copy of your birth certificate (if you were born in Georgia), click here. (dph.georgia.gov/ways-request-vital-record/birth)
  • One passport photo (2”x2”).
  • Proof of identity (picture identification, such as driver’s license, etc.)
  • For children under the age of 16, both parents must sign the passport application except in extenuating circumstances.
